The nose plays a very important role in face beauty as an organ located in the middle of the face, immediately visible at first sight. For this reason, nose surgery is one of the most common aesthetic operations.

The function of the nose should be taken into account during plastic surgery

Our nose, while contributing to the integrity and expression of our face, has also undertaken functional functions. The main tasks of our noses are their olfactory (smelling) function, to be the main route of air passage during breathing, to clean and hydrate the air during passing. When planning aesthetic nose surgery, airway clearance of the person and interventions to be done here should also be considered carefully since a nose that looks beautiful but functionless indicates a failed surgery.

Retrospective and contemporary viewpoint of nose aesthetics

Nose surgery is a procedure that reveals the perspective and basic philosophy of the plastic surgeon. In the past years, when aesthetic surgeries were difficult to reach, it was considered successful to make very artsy, artificial looking and pug noses with tips are up and bones are carved. However, today, our main goal is to create a nose which is in harmony with the other elements of the face, unnoticeably natural as possible and breathable. Of course, the nose should remain intact for life.

A perfect, stereotyped organ is remarkable and incompatible with human nature. After such a surgery, the nose will gather all the gaze upon it and will be the main focus on the face. With a natural nose, the gaze shall focus on the eyes, not the nose. This can only be achieved by forming a nose compatible with the entire face.

The importance of nose in facial aesthetics

During their training, plastic surgeons study not only on ”nose" but also on all facial aesthetics and proportions. Therefore, they make maximum effort to ensure the compliance of the nose to be formed with the other parts of the face, eyes, chin etc. In other words, they count the nose as a part of face, not a single organ and try to achieve harmony. If a nose that is harmonious with the entire face can be created, then the target has already been reached.

We too, in our surgeries, aim to create a nose suitable for the face and expression with the least touch, with minimum intervention to give a natural appearance, rather than re-creating the whole nose. So we focus on what the person needs, at what points touch is needed. We can define this philosophy as “minimally invasive rhinoplasty”.

The desired point to be reached is that, there is no evidence about surgery, the nose is unnoticeable and lost among the other elements of the face.

What is the ideal nose shape?

Although aesthetics can not be explained by exact mathematical values, it will be realized that our concept of beauty is related to symmetry. We generally define what is symmetrical, compatible with composition and familiar to us, as beautiful.

With all these explanations, we can say that the most esthetical and beautiful nose beyond certain values is the nose that looks proportionate and natural to your face. The first condition for the success of a nasal aesthetic should be that it looks natural. Symmetrical, harmonious and natural-looking rhinoplasty that is not understood to be operative is supposed to be successful.

Reasons for rhinoplasty

We can mention two different reasons for rhinoplasty:

(Functional or reconstructive) rhinoplasty for health purposes

Rhinoplasty might be your treatment if you have problems mainly caused by nose, such as some congenital deformities, breathing difficulties due to accidents or various diseases or bumps, nasal congestion, speech difficulties, fractures, snoring, sleep disorders, anosmia or diminished smell and taste perception problem.

(Cosmetic) rhinoplasty for aesthetic purposes

Rhinoplasty is an aesthetic surgery procedure that is performed to complete the facial beauty as a result of the person's dissatisfaction with his/her nose. Rhinoplasty provides various processes such as reducing the height of nasal bridge, correcting the deviated septum or dorsum, reshaping or lifting the tip, reducing the size of the nose, narrowing the vibrissae, reshaping the nostrils etc.

The agenda before rhinoplasty

Before surgery, you should talk to your doctor in detail about what you are uncomfortable with in your nose. Is it aquiline? Crooked? Is the nasal tip droopy?  Does it turn downwards when you laugh? Is the dorsum narrow or wide? Are you having difficulty in breathing?......?

You should also talk about how you want your nose to be? Too small? Ski slope nose? Slim? How will the nostrils be?.........?

Your doctor will tell you which of these requests can be obtained, whether the nose you want will adapt to your face, how you will look after surgery and what should be done to make your nose look natural after surgery.

It is now possible to view more or less how your nose will look after surgery on a computer. Here is my recommendation; be sure that you are in harmony with your doctor at all stages and that you have the mutual language and trust.

Duration of rhinoplasty surgery

Rhinoplasty operation lasts between 1.5-3 hours, according to the interventions to be performed on the patient. While the operations to be performed only at the tip of the nose take a short time, the ones which the whole nose needs to be reshaped, the ones which requires the intervention to the airway and the cases which the nose needs to be supported by cartilage patches take more time.

After rhinoplasty

There is almost no postoperative pain. Even if there is a slight pain, it can be prevented with pain medications.

I advise my patients to stay in the hospital on the day of surgery. Because, at home, it is difficult to cope with the slight leaks, tingles that may occur and the feeling of nausea due to anesthesia, on the first day. In the hospital, the first night is spent comfortably under the control of the physician and nurse.

Rhinoplasty: before/after photos

I suggest that people who are preparing rhinoplasty should try to understand the aesthetic perspective and philosophy of their doctor instead of looking at their pre/post photos, during the examination. Because every surgeon has very good results as well as less satisfactory results. However, the best results and the same photos are shown to those who come for examination. Surely, experience is important. But, the doctor who says that he has always performed excellent surgeries is probably not telling the truth.

So, instead of looking at photo albums of former patients, it is more accurate to describe yourself, your demands and expectations, and to try to understand your doctor's approach and philosophy. You will be more calm and comfortable by chosing a surgeon who will handle the process. Because the pre-operation stress may cause the post-operation process to be the same. Yet, it is possible to walk hand in hand, with a smiling face to the result after the procedures which entered peacefully and comfortably.

Questions from you about rhinoplasty

What is profile-plasty?

Aesthetic surgery specialist does not just look at your nose before the rhinoplasty but examines your face as a whole and, to make your entire face look more beautiful, advises you if there are other parts that needs to be balanced such as chin, forehead and cheeks. The procedures carried out for other areas together with the nose is called "profile-plasty".

If desired, the forehead area can be reduced or filled with injections. If the chin is receding, it can be filled or corrected.

Can filling (botox) be used for nose shaping?

Nasal contouring with fillings is a new and very popular topic. For a long time, we have been shaping the face by injecting fillers into many areas. The same procedure can be used for the nose. Hyaluronic acid containing and long-lasting fillings can be used to camouflage the deformities of the nose.

During this process, it is possible to lightly correct and hide slight nasal humps, asymmetries with a small amount of filler. Nevertheless, it should not be forgotten that this procedure never corrects the actual problem but just slightly camouflage. It is a temporary solution, not a permanent one. It only facilitates hiding minor, minimal problems.

The use of large amounts of fillers to correct major deformities or serious disorders is not recommended because it has certain risks.

Is there an age limit for rhinoplasty?

Rhinoplasty can be performed from the age of 18-20.

Is there bruising and swelling after rhinoplasty?

Small operations only on the tip of the nose create very little swelling. And it is natural for some swelling to occur in the complicated procedures that interfere within the bone. In order to reduce this, necessary interventions are performed during surgery and preventive drugs are given before and during surgery. Most of the swelling and slight bruises under the eyes resolve within a week-ten days.

When are the tampons removed after rhinoplasty?

One of the main fears of people who want to have rhinoplasty is tampons. Legendary hearsays about the tampon removal memories of those who had operated in past is notorious. However, today, long and sticky tampons are not used as before. Instead, very short and non-stick or silicone, self-melting tampons, which allow breathing is preferred. So, the fear of tampons is now history.

Is plaster usage necessary in rhinoplasty? When is it removed?

In cases where the nasal bone is intervened, a support is needed to protect the procedure performed during the first few days. Because the bones can move with in-bed turns during sleep or unconscious touches.

In the past, this protection was provided with gypsum plasters, but nowadays, light splints shaped by heat are preferred. Therefore, there is no need for a plaster cast on the nose after surgery.

The splint is removed after 5-7 days. This is a painless procedure.

How long it takes to return to normal life after rhinoplasty?

After minor interventions, it is possible to return to work within 3-5 days. For complicated procedures, I would recommend 7-10 days of rest to my patients. This period does not have to be spent with absolute bed rest. It is also possible to travel to nearby areas and relax in the holiday destinations.

When does the nose get its final shape after rhinoplasty?

Wound healing, which is a process that starts after every intervention to the body, is completed in 2 years. For the nose to take its final shape, this time is about 6 months-1 year.

Can people with sinusitis get a nose job?

Chronic sinusitis is not an obstacle to rhinoplasty, it can even be treated with this surgery.

Doctors, clinics or hospitals that advertise the prices of any treatment on the internet commit the crimes. For this reason, I would advise you to stay away from the people and institutions who offer surgery prices like a fabricated product without knowing you, without examining you, without knowing what you want.

After rhinoplasty

You can stand up 3-4 hours after the procedure. (Please ask the nurses for help. Do not stand up alone.)

Your nurses will tell you when you can eat and drink.

There will be a plaster on your nose. This plaster will be removed 5-7 days after the procedure. Tapes will be attached instead. You can remove these tapes from bottom to top after 10 days (If you are in Istanbul, we will do this.)

After the procedure, you should use your spray and oinment for 15 days.

On the day after the procedure, you can wash your hair so carefully that no water will touch to your nose.

Bruises may appear on your face. These bruises will fade away in 7-10 days.

Intermittant ice application on your face for 2 days is recommended to reduce edema and swelling.

We recommend you not to laugh and talk too much after the procedure. It might increase swelling.

We do not recommend smoking for 15 days as it negatively affects wound healing.

You can have skin care 1 month after the procedure. (Except for the nasal area.)

You can do light sports 15 days after the procedure. We suggest that you wait 3 months for harder sports (team sports, sports with a ball..)

You can drink herbal teas one week after the procedure.

You can use glasses 3 months after the procedure.

You can swim in the sea/pool one month after the procedure.

You can lie on your side after rhinoplasty. It is necessary to wait 3 months for lying face down.

 

After abdominoplasty (Tummy tuck)

After the procedure, you will stay in the hospital for 2 days.

When you wake up, you will have drains in your abdomen area. These drains will be removed in 2-3 days.

When the drains are removed, you can take a whole body shower.

We suggest you rest on the first day after the procedure. On the second day, you can stand up with the support of a nurse. Slight dizziness is normal in the beginning. Drinking plenty of liquid (water, fruit juice, tea, coffee, ayran etc) is useful as it will increase your blood pressure.

We recommend that you start walking slightly bended forward. You can start walking upright about a week.

We recommend that you wear the corset strictly for at least 6 weeks.

You can lie on your back or side after the procedure. You have to wait about one month for lying face down. All your stitches are  self dissolving and hidden aesthetic stitches. That's why we will not remove the stitches.

The scars appear red for the first 3-6 months but fade away over time and become less visible. Your scars will be hidden beneath your underwear. You may use the scar healing cream recommended by your doctor from the 2nd week.

We do not recommend smoking for at least one month after the procedure. Smoking can disrupt your wound healing and cause scars to look worse.

You can return to your deskjob after 7-10 days. If your job requires heavy physical activity, it's good to wait 15 days.

You should protect the abdominal area from sunlight and UV for 6 months. If you are going on holiday or to the solarium during this period, always use 50F sunscreen and cover your scars with tapes. The less the stitches are exposed, the less scars will remain. Even sunrays passing through a bikini or swimsuit can darken the scars.

You can start fast walks one month after the procedure. 2 months should be waited for heavy exercises.

We do not recommend using blood thinning medication, herbal teas etc. in the first week after the procedure. Do not use any medication other than the ones recommended by your doctor following the procedure. Be sure to consult your previous and ongoing medication.


After liposuction

You will be able to stand up with the support of a nurse 2-3 hours after the procedure.

Initially, a mild dizziness is normal when standing up. Get plenty of liquid (water, juice, tea, coffee, ayran etc) as it is useful to raise your blood pressure.

Fluid leakage into the mattress is normal and anticipated on the first day after liposuction. The next day we will have you cleaned by showering.

Swelling and bruising may occur in the treatment area. This will fade away in 7-10 days.

After the procedure, you must use the corset on you for at least 6 weeks. We recommend you to shower with the corset in the first week. You can easly dry it with a hair dryer.

You can then take a bath by removing the corset.

A tight corset is crucial for shaping your body.  So we'd like to keep it on you as long as possible.

When you feel comfortable after the procedure, you can actively start sports. To start walks as soon as possible will be useful for reducing edema.

Lymph drainage massage is useful as well. You can start after 3 days.

We recommend consuming plenty of water and fluids after the procedure.

It will take 6 months for your body to take its final form. The edemas will decrease over time, your body will be more beautiful every day.


After breast lift

When you wake up, your breasts will be taped and you will be wearing a tight bra.

Drains will be mostly withdrawn in 1-2 days.

Initially, the breasts will be stiff, edematous and erected. Over time it will soften and descend slightly.

With the support of your nurse, you will be able to stand up and eat 3-4 hours after the procedure. Please do not try to stand up without the help of a nurse.

Dizzyness is normal in the first try to stand up. This mostly depends on anesthesia drugs. Drink plenty of liquids (water, juice, tea, ayran, coffee etc). So your blood pressure will rise and you will feel less dizziness. You can lie back or on your side. You should wait at least 2 months to lie on your breasts.

After the procedure, you can take a shower and wash your hair without wetting your breasts. It will be necessary to wait for the tapes to be removed for a full-body shower. (7-10 days on average)

You should use the bra we will give you for at least 1 month. Afterwards, we recommend using a tight sports bra for at least 1 month. In the early period, your breasts may have some swelling, bruising and stiffness. Due to edema, a temporary sensation decrease in the nipple and breast skin is normal. This will disappear over time as the edema reduces.

Since all of your stitches are hidden, melting and aesthetic stitches, they will not need to be taken.

We recommend that you rest at home for the first 3 days after the procedure. Then you can go out and do your light work. You can return to your deskjob after 7 days. It is important not to raise your arms above shoulder level for the first 1 month. Arms should be next to your body, not too wide.

You can start light walks after 3 weeks and running after 1,5 months. A minimum of 3 months should be waited for heavy sports that operate the arm and chest area (tennis, bodybuilding, swimming, skiing, pilates and stretching exercises etc). After 1-1,5 months, you can bath in the sea or pool. But we do not recommend swimming movements.

You should protect the operation area from the sunlight and UV for 6 months. If you are going on holiday or to the solarium during this period, always use 50F sunscreen and cover your stitch areas with tapes. The less the stitches are exposed, the less scars will remain. Even sunlight passing through a bikini or swimsuit can darken the scars.

For the first week, raise the head of your bed with the help of 2 pillows. This will allow your edemas to reduce more quickly.

It is recommended that when you go home after discharge from hospital, take the medicines (antibiotics and painkillers) prescribed by your doctor from a pharmacy and use one box each.

After the dressing and wound healing is completed, you will need to use a scar removal oinment. This oinment should be used twice a day for at least 4 months.

Every woman should have a breast check at least once a year. Do not disrupt your routine controls.

 

After breast implant

You'll be wearing a bra when you wake up. You should use this bra for 3 weeks. After 3 weeks, you can use any bra you want.

After the procedure, you can bath without wetting the breasts and have your hair washed.

Drains will be mostly withdrawn in 1-2 days. Then you may have a full-body shower.

Breasts will be stiff due to edema. It may take 2-3 months to soften. There may be temporary numbness, again, it is due to edema.

There will be tapes under the breasts. You can remove these tapes after 15 days.

The stitches will all dissolve spontaneously. You can start using a scar removal oinment 2 weeks after the procedure.

You should protect the operation area from the sunlight for 6 months. If you are going on holiday or to the solarium during this period, always use 50F sunscreen and cover your stitch areas with tapes. The less the stitches are exposed, the less scars will remain. Even sunlight passing through a bikini or swimsuit can darken the scars.

We do not recommend driving for 2 weeks after the procedure.

We do not recommend smoking for at least 15 days as it negatively affects wound healing. Smoking will cause more scars to be left because it impairs wound healing.

One week after the procedure, you can consume herbal teas, vitamin drugs.

You can do light sports 1 month after the procedure.

Breast implant procedure does not prevent breastfeeding.

Sub-muscular prostheses may be a little more painful. This pain can be felt severe like a stab from time to time, within 6 months to 1 year. It is not something to worry about.

Do not use any other medication than those recommended after the procedure. If there are any medications you have already been using, consult your doctor.

Every woman should have a breast check at least once a year. Do not disrupt your routine controls.
